Your medical records are a legal document owned by Tanner Clinic. To release all or portions of your record to third parties, including yourself, you must complete and sign an Authorization for Release of Protected Health Information form which is available below or at each reception desk.

Can you register for Phreesia before the visit?
A: That’s okay! If you have not completed your registration before the visit, see one of our receptionists, and they will be happy to send you a new link which you can complete in the waiting room. Don’t worry, Phreesia saves information you entered previously.
